Did you know❓ GP practice teams now include care navigators.
Care navigators will direct patients to other professionals within the general practice or other medical professionals such as community pharmacists who can best meet the needs of the patients. Successful care navigation can help direct requests more effectively and speeds up appointments for those who need them.

Social prescriber
A social prescriber helps patients to improve their health, wellbeing and welfare by connecting them to community services which might be run by the council or a local charity. www.
Physician Associate
A Physician Associate is a healthcare professional who, while not a doctor, works under the supervision of a doctor to deliver care and treatment with the general practice team.
General practice nurse
A general practice nurse looks after patients with long-term diseases, offer health screening, and also hold specialised clinics such as immunisations, wound care, and women’s health for smear tests and contraception advice. www.
Pharmacist
A pharmacist provides expert advice around medicines. They support you to make sure the medicines you’re taking are working for you, undertake medicine reviews and offer help for common illnesses. www.
Physiotherapist
Physiotherapists are experts in muscle and joint conditions, also known as musculoskeletal. They provide you with an expert diagnosis and treatment if you are experiencing these types of conditions. www.
Paramedic
Paramedics can assess, examine, treat and manage patients of all age ranges with a variety of acute illnesses from injuries to more serious or long-lasting conditions. www.
Mental health practitioner
Patients with mental health needs may be referred to a Mental Health Practitioner rather than other member of the practice team, as they are specially trained in mental health. www.

If it is most appropriate for you to see a doctor, you will be offered an appointment with a GP. However, the GP isn’t always the best person for you to see.

Hi, I’m Dr xxx, a GP at your practice.
I want you to know that your health is important to us and so is your time, which is why we've recruited additional healthcare professionals at our practice.
So, when you contact us, you’ll be seen by the right person at the right time, as quickly as possible.
The next time you contact us for an appointment, if your need or concern needs to be assessed by a doctor, you will be offered an appointment with one of the practice GPs. But, I want you to know, we are not always the best person for you to see.
If this is the case, you may be offered an appointment with one of our extended healthcare team instead.
The different health and care professionals that work alongside GPs have more specialist skills, knowledge and experience for some health conditions. So please be assured if you are offered an appointment with another member of the team who isn’t a GP, you are being seen by the best person for your needs.
We are all working closely to give you the right care, at the right time, from the right person.
